You will then need to print out, and read the 28 page How To Establish A Self-Directed Traditional IRA document. You must complete, sign and date the Individual Retirement Custodial Account Agreement found on pages 7-11. If you are transferring an IRA from another Trustee/Custodian, you must also complete, sign and date the Traditional IRA Transfer Request Form/Direct Rollover Letter found on page 24.
